The Connected Marriage, LLC is a private-pay practice. As a private pay practice, we do not accept insurance.  If you have have out of network benefits with your insurance plan, we can provide you a receipt to submit to your insurance company for possible reimbursement.

Payment

We accept cash, and all major credit cards as forms of payment. In addition, you may also utilize your Health Savings Account (HSA) and Flexible Spending Account (FSA) for services. Payment is due in full at the time of service.

Cancelation Policy

It is requested that you notify me at least 24 hours in advance if you need to cancel  your appointment.  No shows or late cancellations (less than 24 hours notice) will be required to pay a $75 late fee.

Good Faith Estimate*

Under the Federal No Surprises Act (H.R. 133 – effective January 1, 2022), you have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.


Note: A Good Faith Estimate is for your awareness only. It does NOT involve you needing to make any type of commitment to the length or frequency of therapy sessions.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
  • The good-faith Faith Estimate does not include any unknown or unexpected costs that may arise during treatment. You could be charged more if complications or special circumstances occur.
  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
  • Make sure to save a copy or picture of your Good Faith Estimate.
